Storing and Shipping GLP-1 Medication

GLP-1 medications are temperature-sensitive biologics that arrive at your door in an insulated box, often after a day or two in transit. What you do in the first hour after that box lands — and how you store the medication for the following weeks — genuinely affects whether the drug you are paying for stays usable.

The Short Answer

Refrigerate GLP-1 medication on arrival at 36°F to 46°F (2°C to 8°C), never freeze it, and keep it in its original carton away from light. Manufacturer labels allow a limited period at room temperature — but the exact allowance differs by product, and compounded medications follow the compounding pharmacy's instructions instead. Always go by the label and paperwork that came with your specific medication.

When Your Delivery Arrives

Treat the unboxing as a quality check, not just a delivery. Do this before you put anything away:

  • Open it promptly. Cold packs only hold temperature for so long. If you will be away, arrange redelivery or a hold rather than letting a box sit on a porch.
  • Check the cold pack. Still cold is good. Completely warm on arrival is a reason to call the pharmacy before using the medication.
  • Inspect the medication. Most GLP-1 solutions should be clear and colourless to almost colourless. Cloudiness, discolouration or visible particles mean stop and call.
  • Check for signs of freezing. A vial or pen that has frozen should not be used, even if it has thawed.
  • Confirm the label — medication, concentration, dose and expiry or beyond-use date.
  • Refrigerate immediately unless your pharmacy's instructions say otherwise.

Storage Rules That Apply Broadly

  • Refrigerate at 36°F to 46°F (2°C to 8°C) for long-term storage.
  • Never freeze. Freezing damages the medication irreversibly. Keep vials and pens away from the back wall and the freezer compartment, where refrigerators run coldest.
  • Keep it in the original carton to protect from light.
  • Do not store with the needle attached on pen devices — this can allow leakage or contamination.
  • Keep out of reach of children, and dispose of needles in a proper sharps container.
  • Do not use past the expiry or beyond-use date, and observe any in-use period after first puncture.

Room-Temperature Allowances Vary by Product

FDA-approved GLP-1 products each specify their own limited period at room temperature, and the allowances are not the same across brands. Some products permit a number of days out of the fridge, others a number of weeks, and the rules can differ before and after first use.

Rather than relying on a number you read online, check the storage section of the prescribing information for your product, or ask the dispensing pharmacist. If your medication is compounded, the beyond-use date and storage instructions come from the compounding pharmacy and may be materially different from any brand-name product.

Compounded Medication Is Different

Compounded semaglutide and tirzepatide are not FDA-approved products, and the FDA does not evaluate their safety, effectiveness or quality. That extends to storage: beyond-use dating is set by the compounding pharmacy based on its own formulation, and compounded products sometimes contain additional ingredients — several providers compound with B6 or B3, and some oral formulations include ondansetron. If Something Goes Wrong

Call your provider or pharmacy — do not use the medication and hope — if:

  • The package arrived warm or the cold packs were fully thawed
  • The medication was left out overnight or beyond the labelled allowance
  • It may have frozen at any point
  • The solution is cloudy, discoloured or contains particles
  • A pen or vial is cracked, leaking or damaged
  • The delivery was significantly delayed in transit
